The NIE and the TIE aren't the same thing
The NIE is the foreigner identification number, needed for any financial transaction in Spain. The TIE is the physical card that proves your residence authorisation and carries that same number, along with your fingerprint and the permit's validity dates.

Form EX-15 or EX-17 depending on the case, your passport and a full copy of it, proof of the reason for the application, the form 790 fee paid, a recent photo, and — for the TIE — the favourable authorisation decision.

Appointments are requested through the electronic headquarters and availability varies considerably throughout the year. Once the authorisation is granted, the TIE must be requested within one month of entry or of notification. Showing up with incomplete documents usually means losing the appointment and waiting weeks for another.
